Update my Name

To inform us of a change in name due to marriage/divorce or legal name change please email us with the below documents:

• Slate Change of Details form
• Two forms of identification (i.e. Passport, Drivers Licence, Medicare)
• Certified copy of your Change of Name Certificate, or Marriage Certificate.
