Rollover into Slate from Kiwisaver

If you wish to transfer over your KiwiSaver account into your Slate Account, you will need to first contact your KiwiSaver provider in order to start the transfer process. The form you receive will need to be submitted to your KiwiSaver account provider directly. 

As part of the Kiwisaver form, you will need to include information about Slate Super, including the fund details and Slate Super's bank details, as below:
