An application for financial hardship is assessed on a number of supporting documents. In order to process an application for financial hardship, you must provide the following documents:
- A Centrelink Q230 letter confirming you have been receiving Centrelink payments for 26 consecutive weeks
- Centrelink Income Statement
- A completed Financial Hardship Form
- Confirmation of your identity. This can include:
- A close-up photo of two forms of ID (i.e. Australian Drivers Licence, Medicare Card, Passport)
- A photo of yourself holding your ID accompanied with a close-up photo/scan of your ID
- A photocopy of your ID which has been certified by a Justice of the Peace
- Evidence of current outstanding debts (Overdue Bills, Unpaid Fines, Personal Loans). The debts can also be stated in a Statutory Declaration.
- A rental agreement or official proof of your living arrangements
- A bank statement/transaction listing dated within 3 months of the date of your application being lodged. Please ensure that the BSB and Account Number matches the bank details listed in your Financial Hardship form.
