If you wish to transfer your Slate Super account into a Kiwisaver account, please provide us with a completed copy of the Kiwisaver form and the following documentation:
- Proof that you reside in New Zealand
- Certified copy of your Kiwisaver Statement
- Kiwisaver bank details
- Signed statutory declaration that you now reside in New Zealand
- Certified copy of your ID (Please refer to the Certifying ID guide for assistance)
